The coordinated exercise in the DORDOGNE

A Primary Care Team is made up of several healthcare professionals providing primary care, including at least one general practitioner. Together, they coordinate their actions to improve the health pathways of their patients and reduce social and territorial health inequalities.

A Specialized Care Team brings together a group of health professionals formed around specialist doctors (excluding general medicine).

In an PCT/SCT, the healthcare professional works as a liberal.

The Dordogne has 6 validated PCT :

  • Javerlac et La Chapelle St Robert
  • Villefranche de Lonchat
  • La Bachellerie
  • Le Privilège (Périgueux, Marsac, Chancelade)

4 PCT in project :

  • Bassillac
  • Sarlat
  • Beynac et Cazenac
  • Trélissac

The Dordogne has 8 HCs.

Giving meaning to medical practice, pooling the management of the practice, improving the patient experience means responding to the needs of “Working together” and to current health issues: management of chronic diseases, aging of the population.

This structure helps to fight against medical desertification and the feeling of isolation of health professionals.

A HC can only be created and managed by a non-profit organization, a local authority, a mutual insurance company, a private or public health establishment or a Cooperative Society of Collective Interest.

In a HC, the health professional is an employee.

The Dordogne has 24 MHCs.

An MSP is created by a team of medical and paramedical professionals practicing in private practice who sign a common health project.

Depending on the case, the MHCs benefit from financial support from the public authorities.

The territorial professional health communities meet 2 objectives :

– Improve the organization of city care to deal with the challenges of the ambulatory shift (growth of chronic diseases, demographic tension for certain categories of health professionals)
– Develop the coordinated exercise between the city and the hospital.

Core missions :

– Improved access to care, access to attending physicians and unscheduled care
– Organization of multi-professional pathways around the patient
– Territorial preventive actions
– Response to health crises

Optional missions  :

– Development of the quality and relevance of care
– Accompaniment of health professionals on the territory
